Update Authorized Contact List

In order for us to accept any changes to the ACL and ensure your information is protected and private, we ask that you use one of the following three options and follow the instructions below.
  1. A signed fax on company letterhead from one of the already existing
    contacts or
  2. An e-mail to rm-lcmtierone@ems.att.com from one of the already
    existing contact(s) from the email address on file or
  3. If all the listed contacts are no longer with the company, a fax signed
    by a company officer must be sent on the organization's letterhead
    authorizing the new changes.

To provide you the very best in service, it is imperative that we receive the following information on each contact. Please provide appropriate contact type and ranking for each representative based on the legend below. The requested changes will be completed within one business day.

- Requestor's Name
- Requestor's E-mail
- Company Name
- Customer Number
- First and Last Name
- Primary Phone Number
- E-Mail Address
- Fax Number
- Pager Number
- Alternate Emergency Number(s) (identify mobile, home, etc.)

Contact Type (See descriptions below):
- Administrative
- Billing
- Technical
- Technical Vendor

The order in which individuals should be contacted if more than one contact type of each is listed

  Contact Descriptions
Administrative Company representative authorized to update Authorized Customer List, request remote hands, terminate services, update account information, change organization's name, and initiate trouble/service tickets.
**Enterprise Web Hosting Customers Only - Please specify Data Center Access
Billing Authorized company representative to initiate tickets for billing requests
only. May only change billing contacts and billing address.
Technical Authorized company representative to request remote hands and initiate trouble/service tickets

Technical Vendor Authorized non-company representative to request remote hands
